Health Insurance Chiropractic Policy We provide you with a simple questionaire designed to assist you in finding out exactly how your visits to our office are covered by your insurance company. You will need to contact your insurance provider to complete the questionaire. We ask for payment in full at the time of your visit. We then give you a receipt to send to your insurance company for reimbursement. Please print our insurance questionaire (pdf file) which will help you to verify the CHIROPRACTIC coverage in your policy. Make sure that you bring this information to your first visit.
